normal style='text-indent:28.0pt;line-height:150%'>1.必须符合《政府采购法》第二十二条规定；

normal style='text-indent:28.0pt;line-height:150%'>（1）  具有独立承担民事责任的能力：提供法人或其他组织的营业执照等证明文件（营业照副本、组织机构代码证副本和税务登记证副本，或三证合一的营业执照）或自然人身份证 ；

normal style='text-indent:28.0pt;line-height:150%'>（2） 法定代表人参加投标的必须有法定代表人身份证，法定代表人授权委托人参加投标的必须有法定代表人授权委托书及被授权委托人身份证；

normal style='text-indent:28.0pt;line-height:150%'>（3）具有良好的商业信誉和健全的财务会计制度：

normal style='text-indent:28.0pt;line-height:150%'>具体要求：提供2020年度或2021年财务审计报告（包含资产负债表、利润表、现金流量表及报表附注，注：审计报告要加盖审计机构公章，要提供审计机构的营业执照和执业资格证）或基本开户行的银行2021年至今出具的有效资信证明（至出具之日起2个月有效）或财政部门认可的政府采购专业担保机构出具的有效担保函，并承诺有良好的商业信用和健全的财务制度；

normal style='text-indent:28.0pt;line-height:150%'>具体要求：提供具备履行合同所必需的设备和专业技术能力的证明材料或提供承诺函加盖鲜投标人鲜章； 

normal style='text-indent:28.0pt;line-height:150%'>（5）具有依法缴纳税收和社保缴纳证明的良好记录：

normal style='text-indent:28.0pt;line-height:150%'>具体要求：提供依法缴纳税收（2021年以来任意连续3个月）和社保缴纳证明（2021年以来任意连续3个月）的相关证明材料。未发生缴税情况的，须提供零申报证明，即提供企业所在地税务部门出具的申报证明或加盖税务机关鲜章的申报表，从网上系统打印的需加盖投标人鲜章；提供2021年以来任意连续3个月发生并缴纳社保的有效证明材料；

normal style='text-indent:28.0pt;line-height:150%'>（6）参加本次政府采购活动前三年内，在经营活动中没有重大违法记录的声明：提供参加政府采购活动前3年内本公司及法定代表人在经营活动中没有重大违法记录的书面声明；

normal style='text-indent:28.0pt;line-height:150%'>（7） 诚信资格要求：对列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单的供应商，拒绝其参与报价。查询截图加盖鲜章,提供在“信用中国”网站（包括信用信息首页(法人和其他组织信用信息概述)，失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信名单）、中国 （包括政府采购严重违法失信行为记录名单）查询时间为公告之日至开标截止时间前任意时间；（提供自本项目采购公告发布之日起至投标截止时间止任一时间段的检索查询结果，）提供带网址及版权信息的查询页面完整的彩色查询记录截图（完整清晰）；

normal style='text-indent:28.0pt;line-height:150%'>（8）提供中国裁判文书网无行贿、受贿犯罪证明，无行贿受贿犯罪记录查验证：投标供应商须在中国裁判文书网、检索查询企业以及企业法定代表人是否存在行贿、受贿等犯罪档案记录，若自本项目采购公告发布之日起至投标截止时间止前3年内存在行贿、受贿等犯罪档案记录的将取消其投标资格，提供带网址及版权信息的查询页面完整的彩色截图 （完整清晰）；
